How to Correct Father’s or Mother’s Name on a Birth Certificate in Haryana: Step-by-Step Guide

Correcting father’s or mother’s name is possible in birth certificate in Haryana, but it can be done only within allowable limits.
Let’s first understand what type of corrections are allowed in father’s or mother’s name in the birth certificate of a child in Haryana.
CORRECTIONS WHICH ARE ALLOWED
Surname addition or removal is allowed.
Middle name addition or removal is allowed.
Surname change is allowed.
Middle name change is allowed.
Spelling correction in first name, middle name and surname is allowed.
EXAMPLE:
Sample Father’s Name - ROHIT KUMAR MALHOTRA
Sample Mother’s Name – SHIVANI KUMARI KHANDELWAL
Corrections Allowed in Father’s Name:
ROHIT KUMAR, ROHIT MALHOTRA, ROHIT CHAGGANI or only ROHIT AND ROHEET KOOMAR MALHOTTRAA
Corrections Allowed in Mother’s Name:
SHIVANI KUMARI, SHIVANI KHANDELWAL, SHIVAI MALHOTRA (AFTER MARRIAGE) ONLY SHIVANI AND SHIWANI KOOMARI KHAANDELWAAL
CORRECTIONS WHICH ARE NOT ALLOWED:
A complete name change of the mother or father is not allowed on child’s birth certificate.
Changing Rohit to Sameer is not allowed, and changing Shivani to Varsha is not allowed.
MAIN DOCUMENT ON THE BASIS OF WHICH CORRECTION WILL BE DONE BY THE REGISTRAR
As Per Section 15 Of RBD Amendment Act 2023
“Correction or cancellation of entry in the register of births and deaths.—If it is proved to the satisfaction of the Registrar that any entry of a birth or death in any register kept by him under this Act is erroneous in form or substance, or has been fraudulently or improperly made, he may, subject to such rules as may be made by the State Government with respect to the conditions on which and the circumstances in which such entries may be corrected or cancelled, correct the error or cancel the entry by suitable entry in the margin, without any alteration of the original entry, and shall sign the marginal entry and add thereto the date of the correction or cancellation.”
So it basically means you must provide a document (Aadhaar, birth certificate, passport, PAN, Voter ID, 10th certificate and driving license) that has your name which you want to have on your child’s birth certificate issued prior to the child’s birth.
CHANGE OF SURNAME AFTER MARRIAGE BY MOTHER
The requirement of changing the surname (after marriage) of a mother in her child’s birth certificate is a little different than the requirement explained above.
The main document on the basis of which this change will be made is “MARRIAGE CERTIFICATE”.
Apart from marriage certificate, Aadhaar and PAN Card (having after marriage surname) are also required.

PROCEDURE IN DETAIL
Visit the hospital where the child was born
Submit the application to the hospital with relevant supporting documents for hospital record correction and correction letter.
Once the correction letter and corrected record copy are received visit the local tehsil for affidavits.
Get affidavits made from local tehsil of the father, mother, and two witnesses.
Visit the Municipal Corporation or Municipal Council’s Customer Facilitation Center for application submission once affidavits are ready.
Collect the acknowledgement receipt post application submission
Visit the Birth & Death Registrar's office after two days but within 7 days for original document verification.
Post verification, wait for 30 days for approval.
Once the approval is done, visit Municipal Corporation or Municipal Council’s Customer Facilitation Center for certificate collection.
IMPORTANT THINGS TO KEEP IN MIND
Every hospital’s requirement is different, so please confirm with the hospital their requirement first.
Affidavits should be in proper format; if they are made improperly, then the application will get rejected.
Post application submission, original document verification is mandatory within 7 days. Application will get rejected if not done within 7 days.
