Name addition in the birth certificate can be done only after obtaining the “Without Name Birth Certificate” from the hospital (where baby was born) or from respective zone of Municipal Corporation Delhi.
“Without Name Birth Certificate” issued by Municipal Corporation Delhi will look like this:

DOCUMENTS REQUIRED FOR NAME ADDITION IN DELHI:
Aadhaar of the Father
Aadhaar of Mother
Without Name Birth Certificate
Joint Affidavit of Parents (Notary Attested)
ID Proof of child (Aadhaar or Passport) if available
APPLICATION WILL BE SUBMITTED TO MUNICIPAL CORPORATION DELHI ONLINE
All name addition applications are processed online by MCD. There is a zone-wise facility also available for offline submission of name addition applications.
PROCEDURE IN DETAIL:
Create an account on https://mcdonline.nic.in/ using the mother’s mobile number.
Once account is created, login to your account using same mobile number with OTP.
After logging in at home page, click on SEARCH OLD DATA AND PRINT CERTIFICATE option.
Search the birth certificate of your child by entering:
Registration Number
Date of Birth
Gender
Mother’s Name
Father’s Name
The certificate will appear once the search is complete
Click first on ACTIONS, then on ADD CHILD NAME
A form will appear after that.
Enter child name in blank column of NAME OF CHILD section
Upload the following documents at the end of form in pdf format:
Parents’ Aadhaar
Joint Affidavit of Parents’
ID proof of child (if available)
Once documents are uploaded, click on SUBMIT
After submission of the form, application will be forwarded for approval.
Approval will come in 2-7 days.
Once approval has come, download your certificate.
IMPORTANT THINGS TO KEEP IN MIND WHILE COMPLETING THESE STEPS
Please ensure that you have a good WiFi connection. The MCD website is prone to error at slow speed. Sometimes it fails to capture the complete name of the child due to technical error.
Avoid VPNs while using website.
No changes will be entertained in the child’s name in the future, so be careful with the spelling of the child’s name.
The Affidavit should be properly drafted and should be notary attested.
While entering the name of the child please ensure you don’t end up editing any other section of the child’s birth reporting form mistakenly.
